The above 4 questions are irrelevant to me but I was unable to leave them blank. I had a disappointing experience with this doctor. In my opinion he was not trustworthy or helpful, i.e., I believe he was working in the interest of the insurance company that hired him, not me, the patient. It is likely they would have rated him excellent on all counts. Anyway, I continue to suffer the consequences of a report which I believe did not accurately reflect my condition.
