Dr. Kouchakan made a mistake on paperwork when referring me for a procedure. That mistake put me at serious risk. When I called Dr. Kouchakan's office to cancel my follow up and get my records faxed to me, he proceeded to argue with me for 30 minutes. He claims he didn't make a mistake, and the two clinics who looked at the paperwork both misinterpreted it. If two different teams of nurses and doctors can look at your paperwork and all get something different out it than you what you were intending, you didn't do your job very well. Own up to that, and take responsibility. I was done more harm than good by this doctor's office.
