On the 28th of July 2009, I went to the Hospital of Buckinham located at:155 Rue Mc Leren Est. In Buckinham , Quebec J8L 0C2, I saw Dr. Genevieve Constantineau and made her aware of my severe injury and the pain to my upper back , neck, shoulders, arms and hand. I was unable to turn my neck or lift my arms. I could barely move my neck or my shoulder I was in serious pain (9/10). she was a capital B, she didn't want to mention half of my injury on the WSIB report, It seams to be too much paper work for her to fill-up. which I had an argument with her at that moment about not mentioning my upper back my neck and bilateral shoulder injury. I am now disable and without any income, unable to work because of the extent of the damage of my injury to my upper back and neck. WSIB does not want to cover me because my upper back and neck was not mentioned on the report and now it is impossible for me to prove my case and where my injury came from. since my injury I had nothing but Headaches, frustration, anxiety, depression, because of her and her report to WSIB.

Bad attitude, bad service all around. Too much work for her to fill up CSST OR WSIB reports correctly. She makes up things has it please her. NOT RECOMENDED.

I went in after a work injury. She was verry incosiderate and seam too much work for her to send me for tests or fill up my WSIB report correctly i was in extreame pain to my neck and shoulders and when i asked to see another doctor because of her neglect she treaten to call the police to have me removed. She has destroyed my life.
