Dr. Monton needs to stop over booking and start remembering the patients he had started with 15 years ago. If Dr. Monton's secretary is going to book appointments 6 months in advance, then i think it's only reasonable for his patients to expect that receptionist to call and confirm appointments a few days before they come up. In addition, if Dr. Monton is so booked up that people can't call and get through, then maybe he needs to hire additional staff and put in an extra phone line so that when patients call, they don' t receive an answering machine telling them that the message box for this person is too full to leave a message. How is a person supposed to get through to make an appointment if no one is ever there to answer the phone?
